New Patients

Because your first visit is a great time to establish the valuable foundation of trust, we’ll work hard to make your appointment a success.

Prior to your first appointment, we encourage you to review our website to learn more about us.

What To Expect At Your First Appointment

We encourage you to arrive 15 minutes early to your scheduled appointment so one of our team members can gather your forms and answer any questions you may have.

First appointments typically last an hour or longer and include a professional teeth cleaning, examination and x-rays followed by a consultation meeting with Dr. Fisher. Working together with Dr. Fisher, a personalized treatment plan based on your dental needs and budget will be created and, if necessary, a follow-up appointment will be scheduled.

Our staff looks forward to your first visit and will do everything possible to make you feel at home. Please assist us by providing the following information at the time of your consultation.

  • List of current medications
  • Completed new patient information/medical/dental history and insurance forms
  • Information about any medical conditions (Ex. diabetes, high blood pressure, pre-med antibiotics, etc.)

Please Note: If you have dental x-rays taken within 6 months, please bring them or have them forwarded to our office prior to your appointment. Also parents and/or guardians must accompany patients under the age of 18.

Dental Insurance

For your convenience, we accept most insurance plans and are happy to fill out and submit your insurance forms. However, please note that we do not accept Care Source, Molina or Medicaid. Please bring your current insurance card, policy number and completed insurance forms to your first appointment. If you have any questions, one of our administrative staff members will be glad to help.

If you do not have dental insurance, that is not a problem. Please feel free to call our office staff to discuss any specific questions you may have.
